Can You Donate Blood if You Smoke?
If you smoke and want to give blood, you might wonder whether it disqualifies you. The good news is: yes, you can donate blood if you smoke, whether that’s tobacco or cannabis—but there are some important things to keep in mind to ensure you’re donating safely and responsibly.
Here’s what NHS Blood and Transplant (NHSBT) says about smoking and blood donation in the UK.
Can Smokers Donate Blood?
Yes, smokers are allowed to donate blood as long as they are:
In general good health
Aged 17 to 65 (or up to 70 if you've donated before)
Weighing at least 50kg
Not under the influence of drugs or alcohol at the time of donation
There is no automatic exclusion for people who smoke cigarettes. However, it’s recommended that you don’t smoke immediately before or after giving blood.
Why Should You Avoid Smoking Before or After Donating?
Smoking just before or after donating can increase the risk of feeling unwell. That’s because:
After donating, your blood pressure may temporarily drop
Smoking can worsen dizziness, light-headedness, or nausea
Nicotine can narrow blood vessels, making recovery slower
Smoking after donation may affect oxygen delivery, especially in first-time donors
It’s best to wait at least 30–60 minutes after your donation before having a cigarette, to give your body time to adjust.
What About Vaping?
Vaping is also allowed, and doesn’t automatically stop you from donating blood. However, as with smoking, you should avoid using your vape immediately before or after your donation to reduce any added strain on your circulation or lungs.
Can You Donate Blood If You Smoke Cannabis?
Occasional cannabis use does not disqualify you from donating blood, as long as:
You’re not under the influence at the time of donation
You’re mentally alert and able to give informed consent
You’re otherwise healthy
You must not be visibly high or impaired when attending your donation appointment. If staff believe you are, they may defer you to another day.
Also note: cannabis is still a Class B drug in the UK. While blood services won’t report recreational use, they do require donors to be in a fit state to participate safely.
What If You’re a Heavy Smoker?
If you smoke heavily (e.g. more than 20 cigarettes a day), you might want to let the nurse know at your session. While it doesn’t prevent you from donating, they may offer extra fluids or longer recovery time to make sure you feel well afterwards.
It’s also worth considering that smoking affects overall cardiovascular health, so if you have a related medical condition (like high blood pressure or heart disease), you may be deferred for health reasons—not because of the smoking alone, but due to its effect on your wellbeing.
Final Word
Yes, you can donate blood if you smoke, but you should avoid smoking right before or after your appointment. Whether you use tobacco, vape, or occasionally use cannabis, the key requirement is that you are well, sober, and able to give informed consent.
If you meet the other eligibility criteria and feel healthy, your donation will be just as valuable—and could save lives.
